Nine of ten (90%) cases of AR showed positive staining for inhibin and negative staining for calretinin, PAX2 and PAX8. ![]() Two cases were deferred at the time of frozen section, and one case was initially misdiagnosed as RCC. ![]() All AR cases were incidentally identified (8 cases of donor kidney, 1 radical nephrectomy due to poor renal function, and 1 with abdominal pain). A random cohort of 10 RCC cases was selected for comparison. Results: A total of 10 AR cases was identified the mean age was 54 years, mean size 0.9 cm, and male to female ratio was 1:1.5. Methods: Archival cases of AR were reviewed and compared with a cohort of RCC cases using a panel of IHC stains including PAX2 and PAX8, calretinin and inhibin. In this project, we propose using immunohistochemical (IHC) stains to differentiate AR from RCC. Ectopic adrenal tissue is of no functional significance but may pose a diagnostic challenge in some cases, especially in the context of renal clear-cell carcinoma (RCC) and small needle biopsies. In the kidney, AR can be found in 6% of the general population.
